Job Summary

We are seeking an experienced and compassionate Remote Patient Monitoring-Home Telehealth (RPM-HT) Case Manager Nurse to join our team in Portland, Oregon. As a key member of our healthcare team, you will play a vital role in coordinating care for Veterans with complex health needs, utilizing health informatics, disease management, and telehealth technologies to improve health outcomes and access to care.

Key Responsibilities

• Assess, plan, implement, and evaluate care for Veterans with complex health needs, utilizing a patient-centered approach.
• Coordinate care with interdisciplinary teams, including healthcare providers, social workers, and other stakeholders.
• Provide health education, counseling, and coaching to patients and their families to promote self-management and healthy behaviors.
• Utilize telehealth technologies to remotely monitor patients' health status, identify potential complications, and intervene promptly to prevent hospitalizations and emergency room visits.
• Collaborate with community-based providers to ensure seamless transitions of care and to address patients' social determinants of health.
• Evaluate outcomes of care and participate in quality improvement initiatives to enhance patient care and program effectiveness.
• Maintain accurate and comprehensive patient records, adhering to organizational and regulatory requirements.

Requirements

Basic Requirements

• Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
• Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
• English language proficiency, with ability to read, write, and communicate effectively in English.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN) degree.
• Experience in care coordination, case management, or telehealth nursing.
• 2-3 years of nursing experience, with a focus on complex patient care and population health management.
