About the Role: Case Manager RN - Field

We're seeking an experienced and compassionate Case Manager RN - Field to join our Aetna team in Hudson County, NJ. As a Case Manager RN - Field, you'll play a critical role in serving dual eligible populations by utilizing best-in-class operating and clinical models. You'll work closely with members, providers, and community organizations to address the full continuum of our members' health care and social determinant needs.

Key Responsibilities:

• Conduct comprehensive assessments to evaluate the medical needs of members and facilitate their overall wellness.
• Develop proactive plans of care to address identified issues and enhance short and long-term outcomes.
• Utilize clinical tools and information/data review to incorporate strategies designed to reduce risk factors and barriers.
• Collaborate with supervisors and other key stakeholders to overcome barriers in meeting goals and objectives.
• Present cases at interdisciplinary case conferences and utilize case management processes in compliance with regulatory and company policies and procedures.
• Employ motivational interviewing skills to ensure maximum member engagement and discern their health status and health needs.
• Travel 25-50% of the time to meet members face-to-face in their assigned area, using your own vehicle (mileage reimbursement provided).

Essential Qualifications:

• Minimum 3+ years of clinical practice experience.
• Active and unrestricted RN license in the state of NJ.
• Reliable transportation and willingness to travel 25-50% of the time.
• Reside in Hudson County, NJ.
• Bachelor's Degree in Nursing.

Preferred Qualifications:

• Certified Case Manager (CCM) certification.
• Minimum 2+ years of Care Management, Discharge Planning, and/or Home Health Care Coordination experience.
• Confidence working independently and using tools to collaborate and connect with teams virtually.
• Excellent analytical, problem-solving, and communication skills.
• Bilingual skills preferred.

Pay Range: $69,628.00 - $149,400.00
