Great work you?ll do here: Under the direction of the Occupational Therapist the COTA will provide habilitation or rehabilitation occupational therapy services to clients in their residence.
  • Performs all occupational therapy procedures planned and supervised by a qualified Occupational Therapist in accordance with the plan of care
  • Assists and instructs clients in pushing wheelchair, supporting or lifting, positioning in and out of bed and other activities of daily living
  • Assists in specialized motor or self-help activities, constructs supportive or assistive devices
  • Observes client for signs of distress and adjusts activities accordingly
  • Participates in case conferences, staff meetings and in-service programs as required
  • Prepares clinical notes and progress reports and submits them for review by the occupational therapist to be incorporated into the client?s clinical record
  • Practices with respect for individual, cultural, and spiritual differences
  • Consistently demonstrates competency with occupational therapy skills according to personal and legal scope of practice
  • Evaluates the home environment and make appropriate recommendations
  • Instructs client, family and other team members in proper use of assistive equipment, prosthetic devices and adaptation devices
  • Evaluates effectiveness of teaching and modifies accordingly
  • Promotes client/family autonomy
  • Relays significant changes in status to physician and other members of the team in a time period consistent with client need
  • Documents client charts, records, clinical notes, etc., legibly and according to documentation guidelines and standards
  • Reports any changes in client?s condition to the Occupational Therapist or RN
  • Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company

  • Outstanding presentation, writing and communication skills
  • Promotes the agency through positive representation and communication of its services
  • Strong ability to be the public face of the organization, and serve as an advocate for Home Healthcare, Home Hospice and other service lines
  • Creative thinker who embraces thought leadership
  • Certified as an Occupational Therapy Assistant by the American Occupational Therapy Association
  • One (1) year experience as a COTA in any acute or post-acute setting
  • Current valid license as an Occupational Therapy Assistant (in good standing) in the state(s) of practice
  • Valid driver?s license and automobile insurance
