The Care Team Hospice is seeking an HCHB Integration Application Specialist to support, optimize, and enhance our Homecare Homebase (HCHB) and related application systems across our growing organization.   • Fully remote position
  • Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
  • Occasional flexibility may be needed to support upgrades, implementations, or organizational initiatives

Key Responsibilities

  • Support and optimize Homecare Homebase (HCHB) and related healthcare applications
  • Assist with system configuration, implementation, upgrades, and release management
  • Troubleshoot complex EMR/EHR issues and identify root-cause solutions
  • Support reporting requests and workflow improvements across departments
  • Partner with users to improve operational efficiency and application performance
  • Provide education, training, and ongoing support to application users
  • Participate in merger and acquisition integration efforts as needed
  • Collaborate with vendors and internal stakeholders to support system success

Qualifications• Bachelor’s degree preferred

  • Two or more years of EMR/EHR application support experience within Home Health and/or Hospice
  • Experience supporting healthcare applications such as Homecare Homebase (HCHB), Forcura, Medline, or similar systems required
  • Experience with EMR/EHR implementation, configuration, troubleshooting, and user support
  • Strong understanding of healthcare workflows, including clinical and back-office operations
  • Ability to manage multiple priorities, projects, and deadlines in a fast-paced environment
  • Experience with reporting, system upgrades, and process improvement strongly preferred
  • Strong communication and problem-solving skills
  • Ability to pass a criminal background check
