Part-Time Care Coordinator (Healthcare Startup, Remote, US Hours)

We're a digital health startup offering a nutrition and lifestyle membership for people managing chronic pain, including a 12-week program with lab testing, 1:1 dietitian support, and in-app tracking. We're currently direct-to-consumer and scaling to insurance-covered partnerships with health systems and clinics this summer. We're looking for a reliable, warm, organized Care Coordinator to support our patients and our clinical team (registered dietitians). This is a patient-and-dietitian-facing, phone-and-admin role. A large part of the job is calling patients, so you should genuinely enjoy talking to people, be comfortable making outbound calls, and be patient and empathetic, especially with older adults and people managing health challenges. Hours & rate - ~15 hours per week, $15/hour - You must cover both morning and evening US Eastern (EST) windows so patients can reach us at convenient times (for example, some mornings ~9-11am EST and some late afternoons/evenings ~4-7pm EST) - Remote. You can be based outside the US, but you must be reliably available during EST windows and able to make calls to US patients (we provide a US calling number through our HubSpot CRM) What you'll do - Make weekly check-in calls to patients: see how they're doing, answer questions, and offer help (calls are typically 5-15 minutes) - Onboard new patients by phone: walk them through creating their app account, completing lab tests and in-app questionnaires, and what to expect in and after the program - Re-engage less-active patients - Coordinate scheduling between patients and dietitians (check patient and dietitian availability, schedule the actual consultation) - Leverage our weekly Google Sheet report for the latest status and engagement data - Manage a Google Sheet tracker for patients from specific pilots if required - During every call: take notes and raise support tickets for issues, routing them to the right person - Remind patients and dietitians about consultations What we're looking for - Excellent, friendly phone manner and comfort making outbound calls daily - Highly organized and reliable: you'll prep before each call and own follow-ups and tracking - Comfortable working in a CRM (we use HubSpot; we'll train you if required) and learning new tools - Good judgment about when to escalate: this role does NOT give medical, clinical, or lab advice; you'll learn to recognize those questions and route them to the clinical team - Clear spoken and written English - Available across morning and evening EST windows - Prior experience in patient support, care coordination, healthcare, or customer success - Comfortable working in a scrappy startup environment that is continuously evolving - Comfortable working in a HIPAA-secure environment and very diligent in respecting it - Shows initiative when it comes to proposing process improvements - Excited about contributing to a big mission of making precision nutrition the standard of care for chronic pain and chronic disease in the United States Important, please read before applying This role is primarily outbound patient calls plus coordination and admin. It's steady, repetitive, relationship-based work, not project-based, strategic, or clinical.
