Work Environment

This position operates in a hybrid work environment, allowing flexibility in your working arrangements while ultimately serving patients in a Skilled Nursing Facility with assisted living options. Our facilities are equipped with state-of-the-art therapeutic equipment, which enhances the overall treatment experience and ensures effective rehabilitation outcomes. You will collaborate daily with an experienced team of healthcare providers, sharing knowledge and expertise to optimize patient care.

Key Responsibilities

• Develop and implement comprehensive therapy programs designed to enhance patients’ physical, emotional, and mental health.
• Collaborate closely with multidisciplinary team members—including physicians, occupational therapists, and nursing staff—to craft individualized patient treatment plans.
• Maintain detailed documentation of patient progress, treatment outcomes, and modifications to therapy plans, ensuring seamless communication with the healthcare team.
• Educate patients and their families on therapy techniques, promoting understanding and engagement in the rehabilitation process.
• Stay current with innovative therapy methodologies and maintain licensure requirements and ongoing education to provide the best care possible.

Patient Population / Client Focus As a Physical Therapist at Creative Health Solutions, you will work with a diverse patient population, including individuals recovering from surgery, those with chronic conditions, and elderly residents requiring rehabilitation support. Your interventions will aim to improve mobility, strength, and overall quality of life, making a significant difference in their day-to-day functioning and well-being.

Qualifications

• A valid State Physical Therapy License is required.
• A Bachelor's Degree in Physical Therapy or related field.

Compensation and Benefits
