Facilitating a team approach to care coordination, the full-time Care Coordinator IV - NM Licensed will manage interdisciplinary care plans, conduct comprehensive health assessments, and provide support for members with chronic or complex conditions in a remote work environment. Key responsibilities Facilitates collaboration among the interdisciplinary care plan team to ensure effective and quality care delivery Conducts in-depth health risk assessments and comprehensive needs assessments across various parameters Coordinates care for members with chronic or complex conditions, implementing and monitoring strategies to improve health outcomes Required qualifications Master's degree with 4 years of experience, Bachelor's degree with 8 years of experience, or Associate's degree with 9 years of experience; 12 years of experience may substitute for education requirements Proficiency in Microsoft Word, Excel, and Outlook Experience in analyzing trends using decision support systems Business management skills including cost/benefit analysis and negotiation Knowledge of referral coordination to community and private/public resources Apply tot his job Apply To this Job
