Key Responsibilities • Conduct diagnostic evaluations, diagnoses, psychotherapy, treatment planning, and consulting services, both in-person at Skilled Nursing Facilities and remotely using our Telehealth software. • Complete documentation efficiently in our electronic medical record system (Athena I) from the comfort of your home or office. • Adhere to agency policies and procedures to ensure the highest standard of care. Job Types • Full-time or Part-time • Opportunities for W2 Employees and Independent Contractors • Onsite and Remote options available Requirements • Master's degree in Social Work, Counseling, or a related field, with Arizona licensure as an LCSW, LMSW, LMFT, LAMFT, LPC, or LAC. • National Provider Identifier (NPI). • Professional Liability Insurance. • Credentialing with Medicaid/Medicare is a plus! • Some facilities may require up-to-date vaccinations or appropriate exemptions. • Tuberculosis (TB) Test. • CPR/AED/First Aid Certification. Skills • Basic computer proficiency. • Excellent written communication abilities. • Attention to detail and strong organizational skills. • Strong interpersonal skills and ability to work under pressure. • Fluency in Spanish is a plus!
