Role Description Title: Remote Nurse (L.V.N or R.N) for RPM & CCM Programs Location: Fully Remote (Must be available from 6:00 AM to 6:00 PM Pacific Time) Licensing Requirement: Must hold an active LVN or RN license in the state of California. Candidates with a Compact Multi-State License are highly preferred. Bilingual proficiency (English/Spanish) is a strong advantage. This is a full-time remote role for a Nurse (L.V.N or R.N.) to support Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs at ProMedix. The nurse will play a key role in delivering high-quality care by remotely monitoring patients, coordinating chronic care plans, and ensuring the best possible outcomes for individuals with chronic conditions. Responsibilities include utilizing RPM technology, managing CCM processes, educating patients, and collaborating with healthcare providers. Responsibilities Deliver exceptional care to patients enrolled in Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. Use RPM technology to monitor patients health metrics and provide timely interventions as needed. Coordinate care management programs for individuals with chronic conditions, ensuring alignment with treatment goals. Conduct clinical assessments and develop tailored care plans for optimal patient outcomes. Provide patient education and support, empowering them to manage their health effectively. Collaborate with healthcare providers and clinical teams to ensure seamless care delivery within the RPM and CCM frameworks. Maintain accurate and thorough documentation in compliance with HIPAA, using Electronic Medical Records (EMR) systems. Stay informed and compliant with California and federal nursing regulations. Qualifications Required: Active LVN or RN license issued by the California Board of Registered Nursing (RN) or the California Board of Vocational Nursing and Psychiatric Technicians (BVNPT). Strong knowledge and experience in Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. Proficient in clinical assessment, care coordination, and treatment planning. Familiarity with using RPM technology and EMR systems to monitor and document patient care. Excellent communication and interpersonal skills, with bilingual fluency (English/Spanish) preferred. Ability to work independently in a remote setting while maintaining high levels of organization and accountability. Certification in Remote Patient Monitoring (RPM) is a plus. Preferred: Multi-State Licensure Compact for nurses, allowing practice across multiple states. Schedule and Compliance Minimum Hours Required: 10 hours per week. Compensation: Based on experience and qualifications. Competitive hourly rates will be discussed during the interview process. Must be available for shifts from 6:00 AM to 6:00 PM Pacific Time (flexible scheduling within this timeframe).
