LVN HSS Coordinator - STAR or CHIP - Hybrid in Texas

About the position The LVN HSS Coordinator role at UnitedHealthcare involves working with STAR and CHIP members who have Special Healthcare Needs. This position is crucial in an Integrated Care Model, where the coordinator advocates for members' needs, ensuring they receive appropriate services and care. The role allows for remote work flexibility within Texas, with occasional travel to local offices or community settings. Responsibilities • Assess, plan, and implement individualized care strategies for patients. , • Identify and initiate referrals for social service programs, including financial and community services. , • Manage the care plan throughout the continuum of care as a single point of contact. , • Communicate health-related information to stakeholders to ensure quality coordinated care. , • Advocate for patients and families to ensure their needs and choices are represented. Requirements • High School Diploma/GED (or higher). , • Current, unrestricted LPN/LVN license in the state of Texas. , • 3+ years of clinical experience in a health care role. , • Intermediate level of Microsoft Office proficiency, including Word, Excel, and Outlook. , • Ability to travel up to 10% of the time for team meetings and member engagement. Nice-to-haves • Certified Case Manager (CCM). , • Experience with Texas STAR and CHIP case management. , • Experience working with members with serious mental illness (SMI). , • Experience in managed care. , • Bilingual in English/Spanish or Chinese/Mandarin.
