Responsibilities

  • Retrieves health records and analyzes biometric data to pinpoint gaps in healthcare and deliver disease-specific education and interventions focused on the identified deficiencies under the supervision and direction of a provider, qualified healthcare professional, or registered nurse.
  • Inform and clarify policies and processes for personnel, patients, visitors, and government authorities as necessary.

Qualifications

  • Obtaining a high school diploma or GED demonstrates an individual's commitment to completing their educational requirements
  • CPR certified
  • CNA/HCA certified
  • LPN Preferred
  • Having previous experience for one year in a long-term facility is advantageous
