Your expertise and skills will be vital in helping us identify and address their healthcare needs. Serve as a primary point of contact for members in Champaign, Douglas, and Piatt Counties, providing exceptional customer service and support. Utilize your knowledge and expertise in healthcare to assist members with identifying and addressing their healthcare needs. Conduct thorough assessments of members' medical history and current health status to determine the appropriate level of care and support needed. Collaborate with healthcare providers and other team members to develop and implement effective care plans for members. Maintain accurate and up-to-date documentation of all member interactions and interventions in accordance with company policies and procedures. Monitor and track member progress and provide timely updates to the case management team and other stakeholders. Advocate for members and ensure they receive the necessary support and resources to improve their health and well-being. Stay current on industry trends and best practices in case management to continuously improve the quality of care provided to members. Build and maintain positive relationships with members, providers, and community resources to facilitate effective care coordination.
